Moloka'i Hoe : Registration

REGISTRATION

Required forms for registration are available for downloading and must be submitted by the deadline dates indicated for each form.

Please make sure that each form is completed in it's entirety as it may inhibit your crew from participating.

NOTE: REGISTRATIONS FEES $ 775.00 PER CREW INCLUDES SHIPPING PAYABLE TO: OAHU HAWAIIAN CANOE RACING ASSOCIATION. REGISTRATION AND PAYMENT MUST BE RECEIVED BY 9/24/2010.

	All participants must sign current 2010 Molokai Hoe waivers including passengers and boat captains.
	All participants who are NOT registered members of HCRA must provide proof of traveling insurance provided by your club.
	Insurance is available upon request, however, prior notice must be given so that the appropriate forms and payment transaction may be completed. Please complete the insurance form below and render payment of $ $10.00 per person.
	ESCORT BOATS: Insurance for individual club escort boats are available. Please email: nazarene@molokaihoe.org
	Please include copies of age verification which should include a photo ID ( ie…Driver's license or passport or HCRA ID cards). Failure to provide proper copies may result in disqualification.
	Registration and shipping payments should be made in full prior to shipping your canoe to Molokai. Should payment not be received your canoe will not be shipped.
	ALL REGISTRATION MUST BE COMPLETED ON OAHU OR BY OCTOBER 4, 2010. LATE REGISTRATION WILL RESULT IN A LATE FEE OF $ 250.00 PER CREW AND WE WILL NOT GUARANTEE CREW SHIRTS.

Downloading Files:
Different browsers may provide different means of downloading files. But the more common way of downloading files is to "Right Click" on the links and select from a pop up menu which will appear the appropriate command/menu item to save the file to your PC.

The Intent form should be filled out online to lock-in each of your entries. Please follow-up with the online Entry/Registration form that details specifics, this will insure quick processing of your registration.

PDF/DOC Form Files

You will need Adobe Acrobat Reader to view and edit the downloadable PDF forms

You will need Microsoft Word (ver 1997 up) to view and edit the downloadable DOC forms

NOTE: SIGNATURES
-It is sufficient to type your first and last name on fields for signatures.
But we request that you sign it personnaly as well.

Important Note Concerning the PDF forms below and their ability to be completed on your computer. These forms are designed to allow you to "fill in" your information on your PC so you may print them assuring that the information you provide is ledgable. Unfortunately, depending on the version of Adobe Reader you may be using, the ability to "fill in" your information may not be immediately accessible. If you are using Adobe reader ver 9 and you are unable to "fill in" your information, then please follow the steps below:

1. Open the Acrobat software directly (not through your browser).

2. Select 'edit' and then 'preferences'.
3. In the Preferences window, click on the 'documents' category in the left pane.
4. On the right side of the screen, you should see a section labeled "PDF/A View Mode".
5. Change that setting from 'Only for PDF/A Documents' to 'Never', and then click 'OK'. The form will become fillable.
